1. Efficacy of Berberine in Patients with Type 2 Diabetes
In this pilot study, berberine significantly decreased HbA1c levels in diabetic patients. The effect of decreasing HbA1c was comparable to that of metformin, a widely-used oral hypoglycemic agent.
In combination with other agents, berberine exhibited consistent activities in improvement of glycemic and lipid parameters in diabetic patients. Insulin sensitivity was enhanced by berberine as the HOMA-IR value was reduced by nearly 50%. This effect may be related to fat distribution by berberine because waist and waist/hip of the patients were decreased significantly in the absence of weight change. Interestingly, both fasting and postprandial C-peptides increased significantly in patients when berberine was used together with insulin.
Long-term berberine treatment may improve insulin secretion of the patients with consequent failure of oral hypoglycemic agents.
2. Berberine improves glucose metabolism through induction of glycolysis
In summary, our data suggest that berberine improves glucose metabolism through induction of glycolysis. The increase in glycolysis is likely a consequence of inhibition of glucose oxidation in mitochondria. The elevated glycolysis may be a primary cause for upregulation of glucose uptake in muscle and adipocytes by berberine.
Berberine may improve glucose metabolism via the induction of glycolysis.

3. Effectiveness of phytotherapy in supportive treatment of type 2 diabetes mellitus II. Fenugreek (Trigonella foenum-graecum).
Antidiabetic properties are attributed mainly to galactomannan, 4-hydroxyisoleucin (4-OH-Ile), diosgenin and trigonelline. These substances demonstrate direct antidiabetic properties in clinical studies by increasing insulin secretion (4-OH-Ile), decreasing insulin resistance and glucose resorption from the GIT (galactomannan) and improvement in B-cells regeneration (trigonelline).
Given the large body of evidence and promising results, fenugreek active substances have a potential to become a source of new antidiabetic medication.
4. Effect of Fenugreek Seeds on Serum Metabolic Factors and Adiponectin Levels in Type 2 Diabetic Patients.
Subjects in the fenugreek seed (n=44) and placebo (n=44) groups consumed 10 g/d of powdered whole fenugreek seeds or 5 g/d of wheat starch for 8 weeks. Fenugreek seeds significantly decreased fasting blood glucose (P=0.007) and HbA1c (P=0.0001), serum levels of insulin (P=0.03), homeostatic model assessment for insulin resistance (P=0.004), total cholesterol (P=0.005) and triglycerides (P=0.0001) and increased serum levels of adiponectin (P=0.001) compared with placebo.
Fenugreek seeds improved glucose metabolism, serum lipid profile and adiponectin levels in studied subjects, and may be useful in the control of diabetes risk factors in TD2M patients.
Astragalus Root (Astragalus Membranaceus)
5. Astragalosides isolated from the root of astragalus radix inhibit the formation of advanced glycation end products.
Because advanced glycation end product (AGE) inhibitors such as pyridoxamine significantly inhibit the development of retinopathy and neuropathy in the streptozotocin-induced diabetic rat, treatment with AGE inhibitors is believed to be a potential strategy for the prevention of lifestyle-related diseases such as diabetic complications.
Data suggests that AR and astragalosides may be a potentially useful strategy for the prevention of clinical diabetic complications by inhibiting AGEs.
6. Effect of Astragalus membranaceus extract on diabetic nephropathy
Diabetic nephropathy, a microvascular complication of diabetes, is a progressive kidney disease caused by angiopathy of the capillaries in the kidney glomeruli. Herein, we report a case of a 62-year-old patient with a 30-year history of diabetes, who showed a substantial improvement in diabetic nephropathy on administration of 30 g of Astragalus membranaceus extract per day.
AR shows promise as a therapy for diabetic neuropathy.

7. Management of Diabetes and Its Complications with Banaba (Lagerstroemia speciosa L.) and Corosolic Acid
A number of studies in animals and human subjects using highly purified corosolic acid and corosolic acid-standardized preparations indicate that this component of Banaba exhibits properties that are beneficial in addressing various factors involved in glucose regulation and metabolism, including the enhanced cellular uptake of glucose, improved insulin sensitivity, decreased gluconeogenesis, and inhibited intestinal hydrolysis of sucrose, thereby lowering blood glucose levels.
Banaba may be beneficial in addressing factors associated with glucose regulation and metabolism.
8. Protective effects of Lagerstroemia speciosa on 3-morpholinosydnonimine (SIN-1)-induced oxidative stress in HIT-T15 pancreatic β cells
Lagerstroemia speciosa leaves (LWE) decreased the intracellular levels of ROS and lipid peroxidation, and increased the activities of antioxidant enzymes. These results suggest that LWE has a cytoprotective effect against SIN-1induced oxidative stress in HIT-T15 cells through the inhibition of lipid peroxidation, a decrease in ROS levels and an increase in antioxidant enzyme activity. In addition, LWE increased insulin secretion in SIN-1-treated HIT-T15 cells. Our results suggested that LWE were effective in the treatment of diabetes.
Study results suggest that Banaba shows promise in the treatment of diabetes.
Cinnamon extract (Cinamomum casia)
9. Cinnamon extract improves fasting blood glucose and glycosylated hemoglobin level in Chinese patients with type 2 diabetes.
For thousands of years, cinnamon has been used as a traditional treatment in China. However, there are no studies to date that investigate whether cinnamon supplements are able to aid in the treatment of type 2 diabetes in Chinese subjects. We performed a randomized, double-blinded clinical study to analyze the effect of cinnamon extract on glycosylated hemoglobin A1c and fasting blood glucose levels in Chinese patients with type 2 diabetes. Both hemoglobin A1c and fasting blood glucose levels were significantly reduced in patients in the low- and high-dose cinnamon extract groups, whereas they were not changed in the placebo group.
This study indicates that cinnamon supplementation was able to significantly improve blood glucose control in Chinese patients with type 2 diabetes.
10. Anti-diabetic effect of cinnamon extract on blood glucose in db/db mice
The anti-diabetic effect of Cinnamomi cassiae extract (Cinnamon bark: Lauraceae) in a type II diabetic animal model (C57BIKsj db/db) was studied. Cinnamon extract was administered at different dosages (50, 100, 150 and 200 mg/kg) for 6 weeks.
It was found that blood glucose concentration is significantly decreased in a dose-dependent manner (P<0.001) with the most in the 200 mg/kg group compared with the control. In addition, serum insulin levels and HDL-cholesterol levels were significantly higher (P<0.01) and the concentration of triglyceride, total cholesterol and intestinal alpha-glycosidase activity were significantly lower after 6 weeks of the administration.
Results suggest that cinnamon extract has a regulatory role in blood glucose level and lipids and it may also exert a blood glucose-suppressing effect by improving insulin sensitivity or slowing absorption of carbohydrates in the small intestine.
